Based on the findings of the onsite follow-up inspection, PA Treatment and Healing was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Plan of Correction
|
704.11(b)(1) LICENSURE Individual training plan.
704.11. Staff development program.
(b) Individual training plan.
(1) A written individual training plan for each employee, appropriate to that employee's skill level, shall be developed annually with input from both the employee and the supervisor.
|
Observations A review of personnel records was conducted on August 28, 2019 at the PA Treatment and Healing Scranton facility. During the review, it was discovered that employee #1 did not have an annual individual training plan.
Employee #1 was hired as the project director on June 3, 2019 and was still in the position as of the date of the onsite inspection.
The findings were reviewed with facility staff during the licensing process.
|
Plan of Correction The previous facilities director resigned effective December 4th, 2019. The current lead counselor at the time was then promoted to Facilities Director on January 24, 2020. The results and POC of the previous audit dated 9/5/2019 were only made available to the New Facilities Director on January 10th, 2020. The new Facilities director immediately began to establish a policy to address the deficiencies. Some of which were already being implemented before the Follow up inspection on March 10-11, 2020. All the deficiencies have been addressed and new polices have been implemented on or before May 1, 2020.
0063,
This deficiency was at another location, and under the supervision of individuals who are no longer employed with the facility. The new Facilities director who took over on November 24, 2019 Implemented a new Policy dated 5/1/2020 that all new employees in their first month of employ will complete a training evaluation program to identify which trainings to be implemented in the following year. It is the responsibility of the Facilities Director to oversee and document this new Policy within the first month of the employees' probationary period.

704.12(a)(6) LICENSURE OutPatient Caseload
704.12. Full-time equivalent (FTE) maximum client/staff and client/counselor ratios.
(a) General requirements. Projects shall be required to comply with the client/staff and client/counselor ratios in paragraphs (1)-(6) during primary care hours. These ratios refer to the total number of clients being treated including clients with diagnoses other than drug and alcohol addiction served in other facets of the project. Family units may be counted as one client.
(6) Outpatients. FTE counselor caseload for counseling in outpatient programs may not exceed 35 active clients.
|
Observations During the onsite inspection, the Facility Director provided staff hours and caseloads to the auditor for review. Employee #5 was hired as a counselor on September 8, 2014 and was still acting in that position at the time of the inspection. Employee #5 was reported to have 15 hours per week devoted to their 14 clients on their caseload.
The counselor Full-Time Equivalent (FTE) is determined by dividing the total number of hours the counselor devotes to their clients by the facility ' s work week hours, which at this facility is 40 hours. Then, in order to obtain the counselor ' s ratio, the total number of clients on the counselor ' s caseload is divided by the FTE.
The counselor ' s caseload calculation is as follows: 15/40 = .375 (FTE); 14/.375 = 37.3, which equals to a client counselor ratio of 38:1.
These findings were reviewed with facility staff during the licensing inspection. This is a repeat citation from the September 5, 2019 annual licensing inspection.
|
Plan of Correction This deficiency was addressed as follows; the plan of correction is that this location will not hire Fee for service counselors starting 5/1/2020.
