This report is a result of an on-site licensure renewal inspection conducted on June 5, 2006 through June 9, 2006 by staff from the Division of Drug and Alcohol Program Licensure. The following deficiencies were identified during this inspection and a plan of correction is due on July 6, 2006.

705.24 (5) LICENSURE Bathrooms.
705.24. Bathrooms.
The nonresidential facility shall:
(5) Ventilate bathrooms by exhaust fan or window.

Observations
The exhaust fan did not create a noticeable air flow.

Plan of Correction
Executive Director will contact the Armstrong County Memorial Hospital, who is the owner of the facility, and request that the exhaust fan in the restroom be repaired by their Maintenance staff. The ARC Manor Satellite facility staff will check the status of repair and report this to the Executive Director. The ARC Manor Safety Committee will monitor all physical plant standard compliance with this facility twice a year and will make any corrective action plans as needed. The ARC Manor Satellite staff will also report any problems or issues related to physical plant standards to the Safety Committee and Executive Director. |

(a) Exits.
(1) The nonresidential facility shall:
(i) Ensure that stairways, hallways and exits from rooms and from the nonresidential facility are unobstructed.

Observations
The front door to the facility, which is a required exit, had double keyed dead-bolt which would obstruct exit access.

Plan of Correction
Executive Director will contact the Armstrong County Memorial Hospital, the owner of the facility, and request the double keyed dead-bolt on front door to facility be replaced with a thumb-latch lock. The ARC Manor Satellite facility staff will check the status of repair and report this to the Executive Director. The ARC Manor Safety Committee will monitor all physical plant standard compliance with this facility twice a year and will make any corrective action plans as needed. The ARC Manor Satellite staff will also report any problems or issues related to access of the exits to the Safety Committee and Executive Director. |
