Plan of Correction

715.23(b)(5)  LICENSURE Patient records

(b) Each patient file shall include the following information: (5) The results of all annual physical examinations given by the narcotic treatment program which includes an annual reevaluation by the narcotic treatment physician.
Observations
Based on a review of patient records, the narcotic treatment program failed to document the results of an annual physical examination given by the narcotic treatment program, which is to include an annual reevaluation by the narcotic treatment physician in one of seven patient records reviewed.

Patient #6 was admitted December 15, 2017, and was discharged January 29, 2024. The last physical examination documented in the record was dated on December 13, 2022. There was no annual physical examination completed during 2023.

These findings were reviewed with facility staff during the licensing process.

Plan of Correction
Our Clinical Program Support staff member will utilize a spreadsheet to track due annual physical exams. She will schedule patients ahead of their due date to allow for timely completion. She will inform the Clinical Program Manager if any patient is not in compliance by their due date so this can be addressed with patient and their counselor. The Clinical Program Manage will be responsible for oversight of this process.
