INITIAL COMMENTS

This report is a result of an off-site unusual incident investigation conducted on October 27-31, 2025, by staff from the Bureau of Program Licensure. Based on the findings of the off-site unusual incident investigation, Avenues Recovery Medical Center at Valley Forge dba Valley Forge Medical Center was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.
