This report is a result of a complaint investigation conducted on June 2, 2015 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the complaint investigation, the allegations made against Pinnacle Treatment Centers Pa-V, LLC were unable to be substantiated.
