Observations One personnel record was reviewed for the counselor assistant position on October 11, 2017. The facility failed to document the provision of direct observation for employee record # 5.
Employee # 5 was hired on 3/21/17 as a Bachelor's level counselor assistant, and was promoted to the counselor position on 6/18/17. For these supervision notes, the facility failed to demonstrate that weekly close supervision included an additional hour of direct observation at least once a week.
This finding was reviewed with facility staff during the licensing inspection.
Plan of Correction Beginning 11/17/2017, all future counselor assistant weekly supervision notes will be forwarded to Pyramid Healthcare's Human Resources department by Ridgeview's Clinical supervisor or program director and placed in that employee's personnel file.
