INITIAL COMMENTS

This report is a result of an on-site physical plant inspection conducted for a relocation request. The inspection was conducted on November 20, 2023 by staff from Department of Drug and Alcohol Programs, Bureau of Program Operations. Based on the findings of the on-site inspection, Gateway Rehabilitation Center Squirrel Hill was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.
