INITIAL COMMENTS

This report is a result of an on-site relocation inspection conducted on July 9, 2018 by staff from the Department of Drug and Alcohol Programs, Bureau of Quality Assurance for Prevention and Treatment, Division of Accountability and Program Improvement. Based on the findings of the on-site inspection, Chartiers Community Mental Health and Retardation Services was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:

The nonresidential facility shall:
(5) Ventilate bathrooms by exhaust fan or window.

Observations
The facility failed to ventilate bathrooms with an exhaust fan or window.
A physical plant inspection for the facility relocation was conducted on July 9, 2018. At the time of the inspection, there were no operable exhaust fans or windows present in the facility's restrooms.
These findings were reviewed with facility staff during the inspection process.

Plan of Correction
The restrooms on the second floor have been thoroughly renovated and operational fans have been installed in both. The fans have been tested this morning and are verified as working. |
