The following deficiencies were identified during this supervisory review:

Plan of Correction

704.11(c)(2)  LICENSURE CPR CERTIFICATION

704.11. Staff development program. (c) General training requirements. (2) CPR certification and first aid training shall be provided to a sufficient number of staff persons, so that at least one person trained in these skills is onsite during the project's hours of operation.
Observations
Based on a supervisory review of the licensure inspection conducted on March 2-3, 2015, the facility failed to provide CPR certification and First Aid training so that at least one person trained in these skills was onsite during all hours of the project's hours of operation.

The findings include:

The staff schedule and CPR certification cards for January, February and March 2015 were given a supervisory review on August 6, 2015. The program is a 24 hour residential facility. There was no documentation that a sufficient number of staff persons trained in CPR were onsite during all hours of the project's hours of operation every day for the months of January, February and March 2015. All but three of the residential aide staff had documentation of a completed CPR certification that had been taken as an online course provided by the American Academy of CPR & First Aid, Inc. There was no documentation that the CPR certification course included hands on instruction.

These finding were reviewed with the Business Manager during the telephone interview conducted on August 6, 2015.

Plan of Correction
The Business Manager confirms that most of our staff received online cpr/first aid training, but did not have hands on instruction. The requirement for hands on/face to face instruction has been reviewed with all applicable staff. A certified trainer will be training all applicable staff on 8/11/15 at 2pm, and on that date, we will be compliant. The Operations Manager and Business Manager will ensure that all future staff, as applicable, will be properly trained and certified in cpr/first aid prior to working any shifts alone. This will be overseen by the Project Director.
