Observations Based on the physical plant inspection the recovery house failed to ensure that the interior, exterior, and grounds or yard of the dug and alcohol recovery house clean, safe, sanitary and in good repair and free of hazards at all times. The recovery house has a leak in the ceiling and the hole that it created was not repaired.

Plan of Correction Crossroads Counseling, Inc. fully intends to comply with all requirements regarding physical plant standards. The house we rent recently had a leak in the upstairs bathroom. The landlord repaired the leak immediately but had not yet repaired the access hole that they had to cut into the dining room ceiling to make the repair. They wanted to be sure that the leak was completely fixed. We've confirmed that the leak is completely repaired and reminded them that the repair needs to be finished. The landlord sent their maintenance department out yesterday to begin the repair. Director will follow up weekly until the repair is complete |
