This report is a result of an on-site physical plant inspection that was conducted on February 22, 2017 by staff from the Division of Accountability and Program Improvement to add the activities of outpatient and partial hospitalization to the license. Based on the findings of the inspection, Libertae, Inc was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Therefore, no deficiencies were identified during this inspection and no plan of correction is required.
