The records that were not in compliance are from a new program at the Esper Treatment Center. The new program is an In Home Counseling Program for the Office of Children and Youth. ETC provides the outpatient counseling. To ensure that all clients enrolled in the program are getting the medical histories completed the clients will be given the medical histories to fill out when they are filling out the self reporting history. The intake packets will include the medical history, the self reporting history and the psychosocial history. It is the resposibility of the counselor to ensure that the medical history is completed along with the other preliminary paperwork. When a counselor is aware that their client is going to be discharged sucessfully or unsuccessfully they must complete the aftercare plan prior to discharge. A blank aftercare plan will be put into the clients file and will be filled out by the counselor with the client present. The supervisor will review the file and the aftercare plan prior to discharge to ensure that the aftercare plan and all other discharge paperwork is completed correctly.
