Observations
Based on the review of staff personnel records, the facility failed to maintain the confidentially of all patient records.On January 03, 2017, DDAP staff reviewed staff personnel records and verified a disciplinary action for a confidentiality violation by a nursing staff member. In addition, a letter from the Consumer Satisfaction Team indicated that confidentiality of client information was a primary concern for the clients at SOAR Corporation-Chester.

Plan of Correction
Soar Corp's Regional Project Director shall continue to investigate any future confidentially issues to respond to a grievance or compliant and issue disciplinary action for a confidentiality violation. Moving forward, Soar Corp shall require a annual confidentially training be completed by all employees and the completion of this training shall be documented in the employees HR file. The annual training shall be completed by January of each calendar year. additionally, Soar shall complete a training with all current employees within the next 30 days and the proof of completion shall be entered in the HR file of the employee |
