Observations Based on a review of administrative documentation, the facility failed to provide full and free access to the records of the facility.Personnel records, training records, and fire drill logs were requested for review at the beginning of the inspection. Facility staff informed DDAP Staff that those items were not housed within the facility. During the two day inspection, documentation was requested multiple times. Due to the reluctance by off-site staff to present the requested documents, the department did not have full and free access to the records of the facility. This finding was reviewed with the facility staff during the licensing process.
