Observations Based on a review of personnel records, CPR certification cards and an interview with the project/facility director, the facility failed to ensure that a sufficient number of staff persons were trained in CPR.
The findings include:
Two personnel records and employee CPR certification cards were reviewed on August 5, 2015. Both employees had CPR certification cards which expired in June 2015. The facility failed to ensure that a sufficient number of staff persons were trained in CPR.
The project/facility director was interviewed on August 5, 2015 at approximately 10:30 AM. The project/facility director confirmed both employees had expired CPR certification. The project/facility director reported both employees had taken a recent CPR certification class; but documentation was not available at the time of the review, on August 5, 2015.
These findings were reviewed with facility staff during the licensing process.

Plan of Correction CPR/First Aid initial or re-certification training for 20 Community Services Division staff is scheduled for 8/25/18 at CORA Services. The referenced employees with expired certifications who were retrained at another location in the interim have yet to receive their active training confirmation cards. These two staff will be included in the CORA-based training if their documentation is still outstanding on that date. Training this number of staff will ensure that personnel trained in CPR and First Aid are available during the Agency's operating hours.
Training management procedures have been reviewed by the CSD Managing Director. CPR/First Aid training is scheduled annually for initial and re-certification training needs. The CSD Intervention Services Director has now established an alternate training partner to avoid the potential of scheduling delays as occurred this year due to overbooking.
The CSD Managing Director will review compliance with this training requirement during annual evaluation of staff training plans prior to the end of each fiscal year |
