Observations
Based on a review of client records, the facility failed to have a complete client record on an individual which included information relative to the client's involvement with the project, including follow-up information within 30 days, per facility policy, in one of three applicable records reviewed.

Client #4 was admitted on December 16, 2025, and was discharged on February 18, 2026. The follow-up information documented in the record was dated April 10, 2026, however; per facility policy follow-up was due by March 20, 2026.

This finding was reviewed with facility staff during the licensing process.

The Lead Counselor or Program Director will periodically audit all discharge charts of their supervisee to ensure adherence to this requirement. Prior to this taking affect Program Director will complete an audit of recently discharged client's and present in staff meeting, any 30 day follow-ups that are missing from the client's charts. This information will be presented in staff meeting, and counselors or designees next supervisions by 6/01/2026
