This report is a result of an off-site complaint investigation conducted on August 12 - 13, 2021 by staff from the Bureau of Program Licensure. Based on the findings of the off-site complaint investigation, the allegations against Summit Medical Services were unable to be substantiated. No areas of noncompliance with the applicable chapters of 28 PA Code which pertain to the facility were identified during this investigation, therefore no plan of correction is required.
