Observations
Based on a physical plant inspection on May 20, 2021 at approximately 2:30 PM and an interview with the facility director, the fire extinguishers were not inspected and approved annually be a local fire department or fire extinguisher company.
The fire extinguishers were observed to be last inspected in April 2020 and were due for an annual inspection no later than April 30, 2021.
The findings were reviewed with facility staff during the licensing process.

Facility Director had fire extinguishers inspected by end of day, May 20, 2021. In the future, Facility Director will contact Inspection Company to ensure that they are scheduled to inspect prior to expiration date. |
