This report is a result of an on-site licensure renewal inspection conducted on December 12 and 13, 2006 by staff from the Division of Drug and Alcohol Program Licensure. The following deficiencies were identified during this inspection and a plan of correction is due on January 11, 2007.

709.33(a) LICENSURE Notification of Termination
709.33. Notification of termination.
(a) Project staff shall notify the client, in writing, of a decision to involuntarily terminate the client's treatment at the project. The notice shall include the reason for termination.

Observations
Termination letters were missing in two of three client records reviewed, #7 and 8.

Observations
Consent to treatment forms were not completed at the time of admission in two of eight client records reviewed, #4, and 6. Client record #4 lacked a signed consent to treatment form and client record # 6 contained a consent to treatment form that was signed more than 30 days after the date of admission.
