709.84. Project management services. (d) The project shall develop a written client aftercare policy.
Observations
Based on a review of the Aftercare Plan and Treatment Engagement Plan policy, the Discharge policy, and the Documentation Requirements policy, the project failed to develop a current written client aftercare policy. They include documentation that is no longer available on their current electronic medical record system.

The Aftercare Plan and Treatment Engagement Plan policy indicates an Aftercare Plan is completed if a client refuses a referral to another level of care or is discharged home from any level of care without a referral being made to another provider. At any other time, a Treatment Engagement Plan is completed.

The Discharge policy indicates information will be documented on the clients ' continuing care/transition plan.

The Documentation Requirements policy indicates the Continuing Care/Transition Plan is offered to the patient at the time of discharge with clear instructions for follow-up.

The new policy states, "In the case of premature discharges (Against Medical Advice, Administrative Discharge, Medical Discharge), this must be documented in a progress note and placed in the chart.
