The Individual Session Note is to include the goals and objective for the client, as well as a clickable option to signify specific goal and objective that is being addressed in the individual session. Clinical Director and Owner are responsible for continued monitoring of the deployment of the new form. In the interim all clinical staff are to utilize the Golden Thread Progress Note in the EMR System and not the Weekly Treatment Team Note previously utilized to capture individual counseling session.
