This report is a result of a complaint investigation conducted on November 9, 2015 and a follow-up investigation conducted on November 25, 2015 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the complaint investigation, the allegations against BGI of Brandywine, Inc. were partially substantiated. No areas of noncompliance with the applicable chapters of 28 PA Code which pertain to the facility were identified during this investigation; therefore, no plan of correction is required.
