INITIAL COMMENTS
 
This report is a result of an on-site physical plant inspection conducted for a capacity increase request. The inspection was conducted on May 31, 2012, by staff from the Division of Drug and Alcohol Program Licensure. The facility will be notified by a separate letter of the determination regarding the capacity increase request.
