If they are deficient in training hours, staff were instructed to obtain the number of trainings hours still needed for the year before 12/15/2022.

711.17(a)(3)  LICENSURE Subchapter B.Approval Procedures.Refusal or

711.17. Refusal or revocation. (a) The Department may revoke or refuse to issue a certificate of compliance for any of the following reasons: (3) Failure to comply with a plan of correction approved by the Department, unless the Department approves an extension or modification of the plan of correction.
Observations
Based on a review of client records, the facility failed to comply with plans of correction that were approved by the Department. A plan of correction for not providing an average of 2.5 hours of psychotherapy per month during the patient's first 2 years in treatment was submitted and approved by the Department for the October 21, 2021 and May 20, 2021 annual licensing inspections. Not providing an average of 2.5 hours of psychotherapy per month during the patient's first 2 years in treatment was again found to be a deficiency in the October 24, 2022 through October 25, 2022 licensing inspection. This finding was reviewed with facility staff during the licensing process.

Plan of Correction
Hospital Administration needed to make a change in management at the Outpatient Treatment Program, due to non-compliance with required duties. The Clinical Supervisor of the Outpatient Treatment Program was terminated in 4/8/2022 and a new Director of Behavioral Health Services was assigned to the Outpatient Treatment Program effective 4/12/2022. Also, there was a large turn-over rate of Counselors between 5/2021 and 9/2022, which led to the 2.5 hours of psychotherapy per month not achieving compliance.

In order to achieve compliance, the Director of Behavioral Health Services, audited the charts of each counselor on 10/27/2022 to determine which clients are in need of the 2.5 hours of psychotherapy per month. A meeting was held with the Counselors on 10/28/2022 to distribute their clients in need of this psychotherapy per month. Counselors were instructed to achieve the 2.5 hours per month of those clients immediately.

The Director will then perform chart reviews on those clients as well as any new client to treatment on a monthly basis. The results of these monthly reviews will be presented to the Performance Improvement Committee.
