Observations
Based on a review of the 2009/2010 annual report and an interview with the project/facility director, the governing body failed to make available to the public an annual report which included a statement disclosing the names of officers, directors and principal shareholders, as required by regulation.
The findings include:
The 2009/2010 annual report was reviewed on February 16, 2011. The facility is publicly funded and required by regulation to make available to the public an annual report which includes a statement disclosing the names of officers, directors and principal shareholders. On November 4, 2010, the facility made available to the public the 2009/2010 annual report. The 2009/2010 annual report did not include a statement disclosing the names of officers, directors and principal shareholders.
On February 16, 2011 at approximately 1:30 PM the project/facility director was interviewed. The project/facility director confirmed that a statement disclosing the names of officers, directors and principal shareholders had not been included in the annual report.

Plan of Correction
A statement disclosing the names of the Board of Directors and the Officers will be added to the Annual Report. The Executive Director will attach this statement to the 09-10 Annual Report in the agency's Administrative File by February 28, 2011. The Executive Director will also be responsible for adding this statement to the 10-11 Annual Report when it is created in the fall of 2011. |
