This report is a result of an initial licensure inspection conducted on January 9, 2009, by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the inspection, Gaudenzia Inc., Middleburg Satellite, was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Therefore, no deficiencies were identified during this inspection and no plan of correction is required.
Note: Based upon the lack of any physical plant citations issued during an April 18, 2008 inspection, an on-site inspection was not conducted.
Since this was the initial inspection conducted, not all regulations were reviewed. During future inspections, all regulations will be reviewed for compliance.
