This report is a result of an on-site licensure renewal inspection conducted on May 28th-30th, 2019 by staff from the Department of Drug and Alcohol Programs, Bureau of Quality Assurance for Prevention & Treatment with the inspection also conducted for the approval to use Methadone in the treatment of narcotic addiction. Based on the findings of the on-site inspection, Polaris Renewal Services, Inc- Perryopolis was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.
