This report is a result of a complaint investigation conducted on October 2, 2014 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the complaint investigation, the allegations made against Addiction Medcine and Health Advocate, Inc. were unable to be substantiated.
