Observations
Based on a physical plant inspection conducted on June 19, 2014, the facility failed to ventilate the first floor client bathroom by exhaust fan or window.
The findings include:
A physical plant inspection was conducted with the Facilities Manager on June 19, 2014 at approximately 8:45 AM. The client bathroom, located on the first floor of the main house, did not have a window and the exhaust fan was not operative.
The findings were confirmed by the Facilities Manager during the physical plant inspection conducted on June 19, 2014.

Plan of Correction
On 7/9/14 The Gaiser Center Maintenance Employee under the direction of the Facilities Manager fixed the exhaust fan in the client bathroom on the first floor of the main house. The fan is now operative and the bathroom is ventilated. The Facilities Manager will conduct a monthly inspection of the restrooms to ensure the exhaust fans are operable. These monthly inspections will also be documented accordingly. |
