Observations
Based on the physical plant inspection, the facility failed to maintain each bathroom in a functional, clean and sanitary manner at all times.
The findings include:
A physical plant inspection was conducted on August 10, 2015. It was observed in the cleint restroom black mold on the ceiling tiles above the shower stalls. In addition, one sink had a pipe that was broken and water was leaking from it.
These findings were reviewed with facility staff during the complaint investigation.

Plan of Correction
On August 11, 2015, TCTC Custodial Department cleaned mold off of the client restroom, and then repainted the ceiling. Daily cleaning of the ceiling in the client restroom was added to the Custodial daily checklist.
Water was immediately secured to the effected sink. Teen Challenge Training Center Facilities Department was notified, and repairs were completed, including replacement of the faucet and pipe.
All staff counseled on importance of notifying Teen Challenge Training Center Facilities Department when anything is broken.
