This report is a result of an on-site licensure renewal inspection conducted on May 5, 2015 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Crawford County Drug and Alcohol Executive Commission, Inc. was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection.

(1) A method of selection for membership.

Observations
Based on a review of facility documentation, the facility failed to complete an annual report for the fiscal year of 2013-2014.The findings included:The onsite inspection was conducted on May 5, 2015 and the facility documentation was reviewed on that date. There was no documentation of an annual report for the fiscal year of 2013-2014.These findings were reviewed with facility staff during the licensing process.

Plan of Correction
The Assistant Administrator will be responsible for seeing to it that the annual report is completed for the 2013-2014 fiscal year by June 22, 2015 and annually thereafter. |
