INITIAL COMMENTS

This report is a result of an off-site complaint investigation conducted on June 5, 2020 by staff from the Department of Drug and Alcohol Programs, Bureau of Quality Assurance for Prevention and Treatment.

Based on the findings of the complaint investigation, the allegations against Summit Medical Services were unable to be substantiated. Therefore, no deficiencies were identified during this investigation and no plan of correction is required.
