Observations
Based on a physical plant inspection, the facility failed to maintain each bathroom in a functional, clean and sanitary manner at all times.During the physical plant inspection, a black mold-like substance was observed on the shower floors and walls in the main bathroom.In addition, a cracked toilet seat was observed in the staff restoom. These findings were reviewed with facility staff during the investigation process.

Plan of Correction
On February 9th, facility maintenance removed all stains located in the back shower area using a cleansing agent, and replaced the seat in the staff restroom. The clinical team will monitor the condition of the shower room during daily house inspections. |
