Observations
Based on a review of personnel records, the project failed to demonstrate that acceptable CPR certification has been provided to a sufficient number of staff persons, so that at least one person with acceptable CPR certification is onsite during all hours of the facility's operation. The findings include:Four personnel records were reviewed for the completion of acceptable CPR certification on September 17, 2015. The project failed to ensure that at least one person with acceptable CPR certification was onsite during the following hours of the facility's operation: - Monday, 5:00 pm-7:30 pm- Tuesday, 8:30 am-5:00 pm- Thursday, 8:30 am-5:00 pmThis finding was reviewed with the facility staff during the licensing process.

Plan of Correction
Counelor who did note have correct CPR certification will be leaving the agency as of 10/9/15. The new employee replacing this counselor already has the correct CPR certification. This new employee will be working the exact same schedule as the business hours.
Until counselor is gone, the nurse and director will be covering the office.
