Observations
Based on observation and a physical plant inspection, the facility failed to provide an unobstructed exit from the suite entrance.
The findings include:
A physical plant inspection was conducted on March 3, 2011. A key lock is located on a marked exit from the facility suite. Therefore, this exit is not unobstructed and operable at all times.
During the physical plant inspection, the project/facility director confirmed there was a key lock on the marked exit from the facility suite.

Plan of Correction
Based on this finding that occurred during the physical plant inspection of our Leechburg Satellite site, ARC Manor will be in contact with the facilities manager of the Leechburg Primary Care Center - where ARC Manor is licensed to provide services. ARC Manor will request that the key lock on the door in question be upgraded to a "thumb lock" system, so that the door can be utilized at all times for emergency exit without the need of a key. Until the point that the Leechburg Primary Care Center makes this change, ARC Manor will leave this door unlocked at all times during our primary care operating hours to ensure that it is accessible for exit in an emergency. |
