Plan of Correction On 03/10/2025, the Chief Executive Officer (CEO) spoke with the lead detective of the case who reported any samples from the SANE examination have been sent to the lab for analysis and as of date, no results have been confirmed. The detective reported results could take 5-6 months to be returned.
On 03/10/2025, the clinician on record reported that the information documented in the client's chart regarding the rape kit results was solely provided by the client, and that the hospital where the examination took place had not communicated that information. An opportunity for improvement in medical record documentation was identified, and a clarifying statement was obtained from the clinician regarding this matter. As of this date, French Creek has no evidence that a sexual assault has taken place.
On 3/20/2025, the identified clinician was provided re-education by the CEO regarding the significance of accurately recording information in the medical record. Additionally, the clinician was instructed to thoroughly document the source of any information that is reported or alleged by clients or others.
By 4/10/2025, all direct care staff responsible for entering medical record documentation will be re-educated by the CEO on the importance of ensuring the accuracy and factuality of information in the chart. Additional instructions will be provided on how to properly document the source of any information reported or alleged by clients or others. Training will be completed by 4/10/2025. In the event staff are unable to complete training by 4/10/2025, their time clock access will be de-activated, and the employee will be unable to work until training is completed. Training will be added to a new hire orientation for direct care staff.
Sustainability Plan: The Director of Quality and Risk Management (QRD) will audit 100% of medical record entries related to facility incidents to ensure accurate and factual documentation is present in the chart. Audits shall remain ongoing and reviewed/clarified with this writer and department leaders when concerns are identified.
Results of the audit will be reported monthly to the Committee of the Whole (COWs) and quarterly to the Governing Board who maintains oversight of the facility. In the event of non-compliance, the employee will be re-educated in writing which will be evident in their HR file.
The CEO will provide quarterly staff trainings to direct care and nursing staff on the following:
1) Client Observations
2) What to do in the event of a rape/sexual assault allegation
3) Boundaries Training
These trainings will address the importance of client observations, how to keep clients safe and what to do when a client reports a sexual concern. The first series of these trainings will take place 5/1/2025. The second series will be held on 9/11/2025. Lastly, the 3rd will be held early due to client's increase of depression during the holiday season. The 3rd training will be done December 11th, 2025. These trainings will be done every 4 months thereafter by the CEO.
French Creek has also increased observations of all clients on its co-ed detox unit (floor 1) as of 4/14/2025. (floor where this event allegedly has occurred) Observations on this unit have increased to 20 minutes rather than 30 minutes for detox clients. This change has provided increase staff presence on this co-ed unit which has increased safety of all clients.
On 2/5/2025, the CEO initiated a new process to ensure that vehicle registrations are renewed prior to expiration. A list of vehicles and registration expiration dates was developed, and renewal dates have been added to the CEO and the Director of Environment of Care's (EOC) work calendars. In addition, new monthly checks were implemented on each vehicle, including the inspection of the registration.
Sustainability Plan: An annual quality indicator will be added to the COWs committee for oversight to ensure vehicle registration is renewed timely for each vehicle operated by the facility. Compliance will be reported annually to the Committee of the Whole (COWs) and Governing Board who maintain oversight of the facility.
In the event non-compliance is identified, the Director of EOC shall be re-educated in writing by the CEO, and the vehicle will not be operated until registration is up to date.
On 3/20/2025, the Corporate Director of Quality and Data Management provided education to the leadership team to ensure that medical treatment services must be offered to both staff and clients involved in company vehicle accidents, regardless of fault.
The DHR will be responsible for ensuring that medical treatment is offered to staff, while the DON will be responsible for ensuring medical treatment is offered to clients involved in company vehicle accidents.
Sustainability Plan: The QRD will add a new quality indicator to the facility's Committee of the Whole (COWs) to ensure that medical treatment is consistently offered to both clients and staff involved in company vehicle accidents. The goal is to achieve 100% compliance for each event. Results will be reported monthly to the Committee of the Whole (COWs) and quarterly to the Governing Board who maintains oversight of the facility.
In the event non-compliance is identified, the Director will receive written re-education that will be documented in the employee's HR file and upon notification, when possible medical treatment will then be offered to the staff or client involved in the accident.
