Observations Based on a review of resident records, the recovery house failed to make at least one attempt to notify the resident ' s emergency contact in accordance with subsection (b) immediately and in no event more than 12 hours after the resident decides to end residency or does not return to the drug and alcohol recovery house as expected.
These findings were reviewed with the recovery house staff during the licensing process.

Plan of Correction In order to ensure Harbor Recovery House does not repeat this deficiency, when a resident of Harbor House leaves unexpectedly (for any reason including when Harbor Recovery House staff has asked the resident to leave due to breaking of recovery house rules), the following procedure will be followed:
1. Live-in House Supervisor will do a headcount and verify the absence of the resident. If resident is not found, House Supervisor will contact House Manager.
2. Attempt to contact the resident directly to understand their situation and ensure their safety.
3. Update the resident's record with the time and date of their departure.
4. Contact the designated emergency contact using the phone number in the resident record immediately, and no more than 12 hours after departure.
5. Provide relevant details about the resident's departure without breaching confidentiality.
6. Record the steps taken, including the contact attempts and responses.
7. File the detailed report in the resident's record and notify the resident's primary counselor.
8. Ensure all staff are trained on the notification protocol and understand their roles in the process.
9. Conduct regular drills to ensure the team is prepared for such situations.
10. Regularly review the notification procedures and improve them based on feedback and incidents.
Either the Live-In House Supervisor, the House Manager, or their designee may complete the above steps, but the House Manager is the person responsible to see that this procedure is followed.
This plan of correction took effect on November 13, 2025.
