Based on the concerns arising from COVID-19, The Department of Drug and Alcohol Programs, Bureau of Quality Assurance for Prevention and Treatment, has implemented temporary procedures for conducting an annual renewal inspection.
The inspection will be divided into two parts.
1, an abbreviated off-site inspection, will be conducted off site, and will require the submission of administrative information via email to a Licensing Specialist.
2, an abbreviated on-site inspection, will be conducted on-site, at a later date and will include a review of client/patient records, and a physical plant inspection.
This report is a result of Part 1, an abbreviated off-site inspection, conducted on July 24, 2020 by staff from the Department of Drug and Alcohol Programs, Bureau of Quality Assurance for Prevention and Treatment. Not all regulations were reviewed, the remainder of the regulations, not reviewed during Part 1, will be reviewed at a later date.

Observations Based on a review of personnel records, the facility failed to ensure that employee #5, who was hired as the facility director on July 15, 2020, met the minimum qualifications to be the facility director. The personnel record documented that the facility director has an Associate's Degree; however, the employee only had 3 years of documented experience in a human service agency with no prior experience in supervision of others and program planning.
This finding was reviewed with facility staff during the licensing inspection.

The drill log indicates the time of the drill, any problems encountered, the evacuation route used, evacuation time, number of people in the facility and whether the fire alarm or smoke detector was operative. The new drill log also allows for a more in-depth analysis of the drills and better tracking of any performance improvement plans.
The Clinical Director, Office Manager and Front Desk staff member form the local safety team that is responsible for carrying out the drills. The Office Manager is responsible for completing the drill logs and ensuring any corrective actions are completed and monitored. Our Senior Office Manager is responsible for establishing a calendar of unannounced drills and ensuring the local safety teams complete each drill for a number of different types of emergencies, including fire and evacuation. This finding was reviewed with facility staff during the licensing process.
|
Plan of Correction This error was the result of a change in our staffing model, which changed doctors and other prescribers from independent contractors to employees. Historically, prescribers paid as independent contractors with buprenorphine waivers would keep separate records for their buprenorphine patients. That practice ended when the medical providers were made employees of Kolmac. All of our prescribers are approved to prescribe buprenorphine by SAMHSA. Kolmac has applied for its buprenorphine waiver from DDAP and has ceased prescribing buprenorphine for new patients, pending the appropriate approval from DDAP. The Chief Compliance Officer will meet weekly with the Clinical Director to monitor the census. The primary goal of those meetings will be to bring the census to 150 and maintain it there, while awaiting a decision from DDAP on the approval of additional capacity. The census will be controlled by discharging patients who are no longer actively involved or compliant in treatment and by limiting admissions if necessary.
The Chief Compliance Officer has added this as an item on our Risk Management Plan to ensure follow up and ongoing compliance. |
