Observations
Based on the review of client records, the facility failed to document a completed consent to release information form, to include specific information to be disclosed, in one out of seven records reviewed.
Client #6 was admitted on April 4, 2024, and was discharged on August 7, 2024. Two consent forms both dated April 4, 2024, to a payor and an emergency contact did not specify what information was being disclosed by the provider.
This finding was reviewed with facility staff during the licensing inspection.

Plan of Correction
The Project and Facility Director shall train counselors by 12/16/24 to include specifically what additional information may be disclosed when reviewing and completing consent to release information form. We will also train on the documentation of this information. Facility Director will review files monthly to ensure compliance. This shall be completed by 12/16/24. |
