Observations
Based on a review of the facilities administrative documentation, including papers filed with the Commonwealth, and information sent to the Department of Health and the Office of Legal Council, as well as a conversation with the Facility Director, the project failed to provide the required legal documents for a change of ownership within 90 days of the occurrence.
The findings include:
NET Treatment Services, INC. incorporated with Riverside Care Inc. on January 22, 2010.
NET Treatment Services, INC. failed to provide the required legal paper work for a change of ownership to the Department of Health and the Office of Legal Council within 90 days.
The Facility Director confirmed during conversations on December 9, 2010 and December 16, 2010 that the project erroneously failed to sign and submit documentation of the corporate takeover.

Plan of Correction
All legal paperwork for a change of ownership has been sent to the Department of Health and the Office of Legal Council. Appropriate information was faxed on October 19 2010 to the Div of Program Licensure PA DOH. |
