The training record was requested to be submitted to the Division on August 26, 2009 in order to determine if the training hours were completed for the 2008-2009 training year. The training was reviewed on August 27, 2009. Twenty-five training hours are required for the counselor records requested. The Facility Director and Clinical Supervisor each stated the counselor had the required hours of training, but the training hours documented only 18 hours of training for the 2008-2009 training year.
