INITIAL COMMENTS

This report is a result of a complaint investigation conducted by telephone on December 11, 2012, by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the complaint investigation conducted by telephone on December 11, 2012, The Consortium, was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:

Plan of Correction

715.7(b)  LICENSURE Dispensing or Administering Staffing

(b) Dispensing time shall be prorated for patient census. There shall be sufficient dispensing staff to ensure that all patients are medicated within 15 minutes of arrival at the dispensing area.
Observations
Based on a complaint and an interview with the director of the Narcotic Treatment Program (NTP) at The Consortium, the facility failed to ensure that there was sufficient dispensing staff to ensure that all patients were medicated within 15 minutes of arrival at the dispensing area.

The findings include:

On December 11, 2012 the Division received a phone call from a patient at the facility stating that the make up group session had been cancelled and that when the patients went to the dosing area to receive their medications, there was no staff there to dispense their medications.

A phone call was made to the facility on December 11, 2012 and the director confirmed that the nursing staff had gone for the day prior to the patients arriving to receive their medications. The Nursing staff had been made aware that the 3:15 PM - 6:15 PM make up group session had been cancelled and they then left the facility at 3:00 PM. The facility was able to contact the nursing supervisor who then arranged for nursing staff to return to the facility to dose the patients sometime after 3:30 PM.

Plan of Correction
Responsible Party: The Medical Director, Program Director and Nursing Supervisor are responsible for ensuring adequate dosing coverage during the hours of operation.

Plan of Correction: As of 12/12/12, the program has implemented the practice of ensuring that the Dispensing Window have emergency staff coverage in place until 3:30 pm, when groups beginning at 3:00 pm (3 pm is the latest start time currently) are cancelled. This will give adequate time to ensure that persons scheduled for 3 pm groups, have presented and recieved their dosage for that day.

The Nursing Supervisor, or designee, will also be responsible for attempting phone contact with each member that the cancellation would affect.

Monitoring: The Medical Director will train Nursing Staff and Program Staff on a monthly basis to ensure implemented processes are followed.
