Observations
Based on an inspection of the physical plant, the facility failed to maintain each bathroom in a clean and sanitary manner.
Findings:
An inspection of the physical plant was conducted on October 28, 2011 between the hours of 09:45 and 11:00 in the company of the facility director and division director. The bathroom on the second floor of the facility was not clean and sanitary. The shower and tub enclosure in the bathroom on the second floor of the women's dormitory contained mold and/or mildew.
The facility director and division director were interviewed and acknowledged the condition of the bathroom on the second floor during the physical plant inspection conducted on October 28, 2011.

Plan of Correction
A work order was sent and approved for the installation of a new shower and tub enclosure in the second floor bathroom. The physical plant manager has already hired a contractor and the work is expected to be completed by the end of November. The Program Director will be in contact with the physical plant manager and ensure the work is completed. |
