Observations The facility failed to have a full-time clinical supervisor for every eight full-time counselors employed in the project at the time of the annual inspection on July 11-12, 2016.
The Staffing Requirements Facility Summary Reports for the nine facilities contained within the project were reviewed. There was an equivalent of 12 full-time counselors employed within the project which would require at least one full-time clinical supervisor. The project lists one clinical supervisor, who was also listed as a counselor with a caseload; therefore, they are unable to provide full-time clinical supervision.
These findings were reviewed with facility staff during the licensing process.

Plan of Correction The current Clinical Supervisor was just promoted on June 13, 2016 due to the resignation of the previous Clinical Supervisor. As soon as the current Clinical Supervisor was promoted the Project Director, Facility Director and Clinical Supervisor began the process for selecting a replacement for the open counselor position due to the Clinical Supervisor being promoted from within the agency. During this time only some of the Clinical Supervisor's caseload could be transferred amongst the other counselors. The new counselor was hired effective 7-13-2016. As soon as her training period is complete she will assume responsbility for the Clinical Supervisor's remaining clients (except 5). The transition of the Clinical Supervisor's remaining caseload will be complete and we will be in full compliance with this regulation by 7-31-2016. The Facility and Project Director will assure ongoing compliance with this regulation through monthly supervisions and ongoing monitoring of incoming referrals and counselor caseloads. |
