This report is a result of an offsite complaint investigation conducted between January 26, 2026 to February 5, 2026, by staff from the Bureau of Program Licensure. Based on the findings of the off site complaint investigation, Soulvation Farms was found to not be in compliance with the applicable chapters of 28 PA Code, Chapter 717, which pertains to the recovery house.

§ 717.25(b)(5) LICENSURE Financial transactions
§ 717.25. Financial transactions.
(b) The licensee shall develop and implement written policies and procedures that prohibit the licensee, staff, volunteers or contractors of the drug and alcohol recovery house from:
(5) Directly or indirectly soliciting or accepting a commission, fee or anything of monetary or material value from residents, other related individuals, third-party entities or referral sources, beyond specified rent established in writing at the time of residency.

Observations
Based on resident and staff interviews it was determined the recovery house accepted money from residents beyond specified rent established in writing at the time of residency.

Plan of Correction
The individual is no longer subcontracted with Soulvation Farms as of January 15th, 2026. Going forward, the recovery house will review all policies and procedures with employees, staff, and volunteers and will explain that there will not be any money lent to or received from a resident or employee who was a resident. The house manager will review this policy with the volunteers, staff, or employees to ensure that it is being followed to ensure that this will not recur.
