The nonresidential facility failed to provide verification of a certificate of occupancy from the Department of Labor and Industry or a local equivalent during the survey for the facility located at 226 Donohoe Road, Suite 111, Greenbsburg, Pennsylvania 15601.

Plan of Correction
Outside In was notified by Labor and Industry that the Occupancy Permit application had been returned for modification due to the application being part of another Outside In, Labor and Industry application. Outside In is resubmitting for the Outpatient Clinic, Labor and Industry application by itself. Outside In was informed that the clinic should have the Occupancy Permit in 6 weeks by October 8, 2007. The Facility Director will monitor progress and time frames to ensure full compliance by October 8, 2007. |
