INITIAL COMMENTS

This report is a result of an on-site licensure renewal inspection conducted on October 22 through 23, 2007 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Gpha Al-Assist Behavioral Health Care Center was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection and a plan of correction is due on November 23, 2007.

705.22. Building exterior and grounds.
The nonresidential facility shall:
(4) Store all trash, garbage and rubbish in noncombustible, covered containers that prevent the penetration of insects and rodents, and remove it at least once every week.

Observations
Based on an October 23, 2007 tour of the physical plant, the facility failed to ensure that the dumpster was covered. The dumpster had no lid.

705.22 (4) LICENSURE
Building exterior and grounds.:
In an effort to address the non-compliance of Standard 705.22. the old dumpster was replaced on November 2, 2007, with a new dumpster equipped with a lockable lid.
Maintenance department will monitor dumspters daily to ensure that lids are kept closed and locked.
A random audit of the dumpster will be conducted periodically by the facility manager to ensure that reg. 705.22. is being consistently adhered to.
