This report is a result of an renewal on-site licensure inspection conducted on March 10, 2026 through March 12, 2026 by staff from the Bureau of Program Licensure. Based on the findings of the on-site inspection, EQ5M6701 TNH Sober Living was found to not be in compliance with the applicable chapters of 28 PA Code, Chapter 717 which pertains to Recovery House licensing.

Plan of Correction

§ 717.29(b)(1)  LICENSURE Physical plant standards - Bldg-Exterior-Grnd

§ 717.29. Physical plant standards. (b) Building interior, exterior and grounds. The licensee shall: (1) Keep the interior, exterior, and grounds or yard of the drug and alcohol recovery house clean, safe, sanitary and in good repair and free of hazards at all times.
Observations
Based on an inspection of the physical plant, the recovery house failed to keep the upstairs bath tub in good repair, a leak was identified. These findings were reviewed with the recovery house staff during the renewal inspection process.

Plan of Correction
A Maintenance ticket was created on the day of inspection; the Maintenance Team corrected the leak on 3/13/2026. This was verified by the Executive Director/House Manager.

At the Resident Leads Meeting on 2/18/2026 the Executive Director informed the Resident Leads of a new ticketing system that allows Resident Leads to submit maintenance tickets for the Maintenance Team to review and complete. These tickets alert TNH Leadership?and are triaged by TNH Leadership?so vital safety concerns can be prioritized and tracked.

The York District Manager will complete at least monthly rounds of all houses to identify safety and/or physical plant concerns, beginning 3/16/2026. Identified concerns will be reported via the Maintenance Ticketing System.
