INITIAL COMMENTS

This report is a result of an on-site licensure renewal inspection and methadone monitoring conducted on January 18, 2023 through January 19, 2023 by staff from the Department of Drug and Alcohol Programs, Bureau of Program Licensure. Based on the findings of the on-site inspection, Narcotic Addict Rehabilitation Program of Thomas Jefferson University was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:

Plan of Correction

715.14(a)  LICENSURE Urine testing

(a) A narcotic treatment program shall complete an initial drug-screening urinalysis for each prospective patient and a random urinalysis at least monthly thereafter.
Observations
Based on a review of patient records, the facility failed to complete and document a random urinalysis, for each patient, at least monthly in one of seven patient records reviewed.

Patient #3 was admitted into the outpatient maintenance activity on January 12, 2021 and was active at the time of inspection. There was no documentation of that a random urinalysis was completed during the months of October 2022 and November 2022.

The findings were reviewed with facility staff during the licensing process.

Plan of Correction
The Executive Director will talk with the nursing coordinator and nursing staff to ensure that all NARP patients receive a urinalysis on a monthly basis. In addition, patients who receive Buprenorphine will be instructed to see the nursing window to receive their monthly urinalysis prior to their appointment with the medical physician or prior to receiving their next prescription to ensure that these patients receive their monthly urinalysis. This plan of correction will begin effective immediately.
