The following deficiencies were identified during this inspection:

Plan of Correction

704.12(a)(6)  LICENSURE OutPatient Caseload

704.12. Full-time equivalent (FTE) maximum client/staff and client/counselor ratios. (a) General requirements. Projects shall be required to comply with the client/staff and client/counselor ratios in paragraphs (1)-(6) during primary care hours. These ratios refer to the total number of clients being treated including clients with diagnoses other than drug and alcohol addiction served in other facets of the project. Family units may be counted as one client. (6) Outpatients. FTE counselor caseload for counseling in outpatient programs may not exceed 35 active clients.
Observations
Based on a review of the Staffing Requirements Facility Summary Report, the FTE caseload exceeded 35 for employee #6.

Employee #6 works full time as a counselor and currently has 37 clients on their caseload. The caseload calculation is as follows: 40 (hours per week with D/A clients) divided by 40 (hours in the work week) = 1 (FTE). 37 (clients on caseload) / 1 (FTE) = 37. 37/1 is employee #6's caseload and it exceeds the maximum caseload of 35/1.

These findings were reviewed with facility staff during the licensing process.

Plan of Correction
There was an error in the calculation of determining the optimum caseload for employee #6. In error, employee #6 is over her allowed caseload by 2 individuals. The Wexford office has become busier due to increased needs for services during COVID 19. The agency recently brought on a new counselor, employee #8, whom has ample room on her caseload for clients. Facility Director will be responsible for reducing caseload of employee #6 back to 35 active patients. It will be the responsibly of the Facility Director to distribute the appropriate clients to the new counselors caseload. Furthermore, Facility Director with the help of the Wexford Clinical Supervisor will monitor the number of active clients on employee #6 caseload to ensure she does not go over 35 in the future.
